Process
You will complete intake forms to assess concerns and needs before the evaluation.
We start with an evaluation focusing on the concerns you have. Then we create a plan of care including goals and frequency. Frequency is determined by need for skilled interventions from the occupational therapist and is at the discretion of your therapist.
